- Value and discounts are based on Lake Almanor Dental’s standard professional fees (UCR).
- Our plan is not insurance or a qualified plan under the Affordable Care Act.
- The membership plan includes a 30-day money-back guarantee. You can receive a full refund up to 30 days after purchase if no services or discounts have been used.
- Payment for treatment(s) that are not included in the plan is due at the time of service.
- Treatment fees will be at the reduced rate of 10% off standard fees when used in conjunction with a 3rd party payment plan (ex. Cherry, Carecredit or other payment arrangement).
- The membership plan & Insurance: Insurance plans may be used for additional treatment but not for any procedure paid for with the membership plan, including any procedures that are discounted (10-20% savings) with the membership plan. If insurance is used, the procedures will be billed at the current standard fees.
- The membership plan does not cover procedures completed by specialists outside our practice.
